What is extensor tendonitis?

Extensor tendonitis is an inflammation of the tendons joining the muscles at the front bottom of the leg to the toes. The inflammation may also spread to the sheaths in which the tendons run.

The two main extensor tendons are abbreviated to EHL and EDL. The latter connects to the big toe and EHL to the other 4 toes. The tendons run over the top of the foot. Their job is to pull the toes up as a step is started and then allow them down again as the foot is planted once more.

The sufferer will experience pain across the top of the foot, some swelling in the same area, pain in walking, pain when pressing the top of the foot and some pain when passively stretching the tendons.

The condition is often caused simply by too much use of the tendons and the muscles to which they connect. Sports enthusiasts are prime candidates as too much running or walking is an obvious cause.

Running on hills is particularly likely to cause this problem, with the muscles straining one way and then the other throughout the run. Running on icy roads can have a similar effect.

Runners can also exacerbate the situation by lacing their shoes too tightly over the top of the foot, bringing unnecessary pressure to bear before even taking a step. The tendons are near the surface of the foot and lie above bones. Tight lacing just squeezes them onto the hard bones. That pressure will be amplified as running takes place.

A similar condition can arise in the tendons serving the fingers. Again small tears in the tendon fibres are caused by overuse and inflammation results. Excessive work involving lifting and twisting is often to blame and gardening is a classic culprit for this injury.
